Motor Carrier Transportation Program

Required submission materials: - ANV Transportation Application. - ANV Trucking Submission input workbook with front sheet, units, drivers, commodities/IFTA, losses, and CAB screenshots. - Target premium per unit by line of business. - ACORD 137. - State-specific ACORD UM where required. - Currently valued loss runs. - IFTA reports for long haul, 4 quarters. - Program Excel vehicle list. - Program driver list. - MVRs for all drivers. Driver and underwriting rules from the workbook: - Driver must be eligible to operate a commercial interstate motor vehicle under FMCSA requirements. - Valid CDL required for the type/size vehicle operated. - Minimum driver age is 23. - Driver must have continuously held a valid CDL from current state of residence for 6 months. - Drivers over age 65 are considered based on MVR and accident criteria; a current long-form physical is required. - At least 2 years of verifiable continuous experience in comparable commercial vehicles is required. - Drivers licensed in states different from the insured domicile require explanation. Key ineligible exposures include for-hire transportation except limited circumstances, bi-national drivers not properly licensed/authorized in the US, Mexico/Canada border crossing, bulk hazardous materials, armored cars, auto haulers, US Mail haulers except referral, pneumatic/liquid/beverage tankers, mobile homes/toters, sand and gravel, fracking materials, freight brokerage/truck brokerage, livery/passenger transport, NEMT/emergency services, APD-only requests, livestock/live animals except lab animals, logging/lumber/pulpwood except finished lumber, coal/fuel haulers, oversize/overweight loads, cranes or attached mobile equipment, towing/wrecker operations, household/office movers, municipal fleets, salvage/scrap metal, trailer-only, and trailer interchange policies.
